Step 2. Claim Form and Documentation
Complete the Home Claim form
You will also need to attach the documents or collect the information listed below which are required to back up your claim.
Please see the documents/information required for each type of claim below, and send them to us attached to your completed and signed home claim form. You can use any of the following methods:

| Photos of damage including any damage caused following immediate emergency repairs plus photos of the room in which the damages occurred and any other rooms affected. |
| A report tracing source of leak by your appointed plumber or competent consultant |
| Repairs estimate of any damages caused directly or indirectly by the escape of water |
| Detailed architect’s report by your appointed architect if there is major structural damage to your property |
| Details of Third Party and respective insurers if escape of water is from neighbouring property and not from your own, or your accident has damaged neighbouring property |
| Please do not dispose of any damaged items |
| Storm damage |
| Photos of damage of any items or areas within your property that got damaged during the storm |
| Technical reports of any items within your property that got damaged during the storm |
| Repairs/replacement estimate of any items or areas within your property that got damaged during the storm |
| Detailed architect’s report by your appointed architect if there is major structural damage to your property |
| Please do not dispose of any damaged items |
| Lightning Surge Damage |
| Technical reports of any items within your property that got damaged by lightning surge |
| Repair or replacement estimate of any items within your property that got damaged by lightning surge |
| Please do not dispose of any damaged items |
| Theft or loss of items |
| Original police report or security report issued at location of theft/loss |
| Detailed list of stolen/missing items if more than one item has been stolen/lost |
| Proof of ownership of missing items such as receipts, guarantees, booklets, manuals, boxes, chargers etc. |
| Recent valuation in respect of valuables including jewellery |
| Photos of rooms in the state you discovered them, in respect of theft from your residence |
| Photos of stolen items in respect of jewellery, even events during which they have been worn |
| Damage to glass & sanitary ware |
| Photos of damage of the damaged glass/sanitary ware plus room in which it is located |
| Repairs/replacement estimate of the damaged glass or sanitary ware which should include dismantling and refitting costs |
| Please do not dispose of any damaged items |
| Damage to electronic equipment |
| Technical Report |
| Repairs or replacement estimate if not already quoted in the above mentioned Technical Report |
| Photos of damaged item in the event of screen cracks or such damages, including photos of the whole unit |
| Please do not dispose of any damaged items |
| Loss of frozen foods |
| Photos of thawed food prior to disposal showing item clearly so that cost can be established |
| List of thawed food including prices and where available, purchase receipts |
