Step 1. What you need in the event of a Pet claim

Fill in the pet claim form

Download the pet claim form. Once it has been completed by your vet, send it to us by email on insure@atlas.com.mt or by post to Atlas Insurance PCC Limited, 47-50 Ta’ Xbiex Seafront, Ta’ Xbiex XBX 1021. You can also drop it in by hand at any one of our branches if this is more convenient.

Check that you have the relevant information and documents needed for the type of claim you are lodging. Please ensure that you make every effort to collect and present the below documents, as missing proof of loss may cause problems in the processing and settlement of your claim.

For Veterinary Treatment

  • Relevant section of claim form to be completed by vet
  • Receipts for relevant vet treatment
  • Preauthorisation from us is to be obtained for overnight stays in clinics or any surgery involving general anaesthetic

For Accidental Death

  • Medical Certificate stating Cause of Death to be issued by examining vet (even in the event that your pet has to be put down)

Advertising & Reward

  • Police Report notifying your loss
  • Inform the Animal Welfare Depertment and other relevant animal rescue centres
  • If possible, use social media to get help to find your pet
  • Before you make any offers of a reward, please get authorisation from us
  • Receipts for any costs that you have incurred, as well as any reward paid out

Third Party Liability

  • If an incident occurs for which you think you may be held liable, you must notify us immediately. You must not admit responsibility but always refer to us.
  • Any subsequent correspondence received – legal or otherwise – must be forwarded to us immediately.

Boarding Fees/Daily Minding

  • Relevant section of claim form to be completed by your doctor
  • Receipts from the boarding home or your pet minder
  • Discharge Certificate from the hospital showing your date of admission and subsequent discharge

Holiday Cancellation

  • Booking Invoice (or such supporting document) showing date of booking, dates of travel, all expenses and any unrecoverable charges
  • Medical Report from vet stating life threatening condition of your pet or requirement of immediate surgery
